Step 1:
Book an Appointment
Our doctors are committed to providing safe alternative care that can be managed and maintained by the patient. Please be advised we will provide the appointment location or telehealth link once it has been scheduled and confirmed to ensure the confidentiality of our physicians and patients.

PAYMENT POLICY
There is a $75 out-of-pocket fee to successfully schedule an appointment online with one of our licensed physicians. A remaining balance of $100 must be paid the day of your scheduled appointment.

CANCELLATION POLICY
The $75 out-of-pocket fee required to book a session is non-refundable if the patient must cancel. One session reschedule will be permitted.
Step 2:
Complete Intake Form
Once you have scheduled your appointment, you will need to complete an intake form before meeting with your physician. A link to the form will be provided to you in your confirmation email.
